The combination of bortezomib, melphalan, prednisone, and thalidomide (commonly abbreviated as VMPT) is a four-drug regimen used primarily in the treatment of multiple myeloma, especially for patients who are not eligible for autologous stem cell transplantation. Bortezomib is a proteasome inhibitor that disrupts protein degradation in cancer cells; melphalan is an alkylating agent that crosslinks DNA; prednisone is a corticosteroid with anti-inflammatory and immunosuppressive effects; and thalidomide has immunomodulatory and anti-angiogenic properties. Together, these agents act synergistically to increase response rates and prolong progression-free survival compared to regimens without all four drugs. Clinical trials have shown that VMPT followed by maintenance therapy with bortezomib-thalidomide improves complete response rates, progression-free survival, and overall survival in newly diagnosed multiple myeloma patients who are transplant-ineligible[1][2][3][4][8].
