Boswelan is a standardized oral extract of frankincense (Boswellia serrata) resin, primarily composed of boswellic acids such as acetyl-11-keto-β-boswellic acid (AKBA). Developed by the Universitätsklinikum Hamburg-Eppendorf, it is being investigated for its anti-inflammatory and immunomodulatory properties in the treatment of relapsing-remitting multiple sclerosis (RRMS) and clinically isolated syndrome (CIS). The drug acts by inhibiting several inflammatory pathways, including 5-lipoxygenase (5-LOX), microsomal prostaglandin E2 synthase-1 (mPGES-1), and IL-1β-mediated IRAK1 signaling, while also modulating IL-17-producing T cells. In the Phase IIa SABA trial, Boswelan demonstrated safety, tolerability, and potential efficacy in reducing MRI-detected brain lesions and clinical disease activity.
