Drug intelligence / Profile preview
Intradetrusor Injection of Type-A Botulinum Toxin refers to the localized administration of botulinum toxin type A (most notably onabotulinumtoxinA, marketed as Botox) directly into the detrusor muscle of the bladder. Developed primarily by Allergan (now AbbVie), this therapeutic approach is indicated for the treatment of overactive bladder (OAB), including idiopathic overactive bladder and neurogenic detrusor overactivity, in patients who have had an inadequate response to or are intolerant of anticholinergic medications. Mechanistically, botulinum toxin type A is a neurotoxic protein that binds to high-affinity receptors (such as synaptic vesicle glycoprotein 2, or SV2) on presynaptic nerve terminals. Upon internalization, its light chain acts as a zinc-dependent endopeptidase that cleaves synaptosomal-associated protein 25 (SNAP-25), a key component of the SNARE complex. This cleavage prevents the exocytosis of acetylcholine and other neurotransmitters into the synaptic cleft, thereby inhibiting detrusor muscle contractions, reducing bladder hyperactivity, and alleviating symptoms of urgency, frequency, and urinary incontinence.
