Overview

This is a combination therapy consisting of botulinum toxin type A, botulinum toxin type B, and phenol. It is used primarily for the management of spasticity in patients with conditions such as cerebral palsy and other neurologic disorders. Botulinum toxins (type A and B) are neurotoxins that inhibit acetylcholine release at neuromuscular junctions, resulting in muscle relaxation. Phenol acts as a chemical neurolytic agent, causing denervation by protein coagulation when injected near nerves or motor points. The combination allows for treatment of multiple muscle groups during a single session while staying within safe dosing limits for each agent individually. This approach increases flexibility in managing multifocal spasticity and can reduce costs compared to using higher doses of botulinum toxins alone[1][2][3][6]. The agents are typically not injected into the same muscle or compartment due to concerns about potential interactions (e.g., phenol denaturing the toxin)[6]. Adverse effects are generally infrequent but may include local pain, temporary weakness, or rare systemic reactions[1][5].
