Botulinum toxin type B is a purified neurotoxin produced by fermentation of Clostridium botulinum type B. It is used primarily to treat cervical dystonia (a neurological disorder characterized by involuntary contractions of the neck and/or shoulder muscles) and sialorrhea. The drug acts at the neuromuscular junction to produce flaccid paralysis by inhibiting acetylcholine release. Its mechanism involves binding to synaptic vesicle-associated membrane protein (VAMP/synaptobrevin), which prevents neurotransmitter release through cleavage of essential polypeptides for vesicle docking and fusion. Botulinum toxin type B is marketed under brand names such as Myobloc (in the US) and NeuroBloc (in Europe). It has demonstrated efficacy in patients with cervical dystonia, including those resistant to botulinum toxin A[1][3][5][8].
