Overview

Bovine calf intestinal alkaline phosphatase (BIAP) is an animal-derived therapeutic enzyme extracted from the intestinal mucosa of bovine calves. Developed by AM-Pharma, BIAP was designed to target mucosal and systemic inflammatory conditions by dephosphorylating and detoxifying lipopolysaccharide (LPS) and extracellular nucleotides (such as ATP, ADP, and AMP). By neutralizing these pro-inflammatory mediators, BIAP prevents the activation of Toll-like receptor 4 (TLR4) and subsequent downstream inflammatory cascades. AM-Pharma successfully evaluated BIAP in Phase 2 clinical trials for sepsis, sepsis-associated acute kidney injury (AKI), and moderate-to-severe ulcerative colitis. However, to mitigate potential immunogenicity and safety concerns associated with animal-derived proteins, development of the bovine formulation was discontinued in favor of AM-Pharma's proprietary recombinant human alkaline phosphatase (recAP, ilofotase alfa).
