Bovine rescue alkaline phosphatase (bRESCAP) is an exogenous, bovine-derived intestinal alkaline phosphatase (bIAP) enzyme developed by Alloksys Life Sciences in partnership with Amsterdam UMC. It is designed to act as a biological response modifier to prevent and treat systemic inflammatory response syndrome (SIRS), ischemia-reperfusion injury (IRI), and organ damage in critical care settings. bRESCAP functions by dephosphorylating and detoxifying major pro-inflammatory mediators, specifically extracellular nucleotides (such as ATP, ADP, and AMP) and bacterial lipopolysaccharides (LPS). By converting extracellular ATP directly into adenosine, bRESCAP reduces purinergic receptor-mediated inflammatory signaling and promotes tissue-protective adenosine receptor signaling. Additionally, dephosphorylation of LPS prevents its activation of Toll-like receptor 4 (TLR4). bRESCAP is being evaluated in Phase 2 clinical trials for the prevention of delayed graft function (DGF) in deceased donor kidney transplantation (the RAPhID/APhIRI program), as well as for cardiac surgery-associated acute kidney injury, severe burns, and other inflammatory conditions.
