BPaLMZ is an investigational five-drug combination antimicrobial regimen being evaluated for the treatment of tuberculous meningitis (TBM), particularly in patients co-infected with HIV. The regimen consists of bedaquiline, pretomanid, linezolid, moxifloxacin, and pyrazinamide. Bedaquiline is a diarylquinoline that inhibits mycobacterial ATP synthase; pretomanid is a nitroimidazole that inhibits mycolic acid biosynthesis and causes respiratory poisoning; linezolid is an oxazolidinone that inhibits protein synthesis by binding to the 50S ribosomal subunit; moxifloxacin is a fluoroquinolone that inhibits DNA gyrase and topoisomerase IV; and pyrazinamide disrupts membrane potential and inhibits fatty acid synthesis. This combination is being studied by the University of Minnesota in a Phase 2 clinical trial to compare its efficacy and safety against the standard RHZE (rifampicin, isoniazid, pyrazinamide, and ethambutol) treatment, with the goal of improving outcomes in TBM, which typically has high mortality and morbidity.
