This experimental treatment protocol, often referred to as In Vitro Activation (IVA), is designed to treat infertility in women with primary ovarian insufficiency (POI). Developed and studied by the National Foundation for Fertility Research, the procedure involves the laparoscopic removal of ovarian cortical tissue, which is then fragmented into small cubes to disrupt Hippo signaling. These tissue fragments are incubated in vitro for 24 to 48 hours with two specific pharmacological agents: **bpV(HOpic)**, a potent inhibitor of PTEN (phosphatase and tensin homolog), and **740Y-P**, a PI3K (phosphatidylinositol-3-kinase) activator. Together, these compounds synergistically stimulate the PTEN/PI3K/Akt/mTOR signaling pathway, which triggers the awakening and growth of dormant primordial follicles. Following the in vitro treatment, the activated ovarian fragments are autografted back into the patient, typically beneath the serosa of the fallopian tubes, to allow for follicle maturation and subsequent egg retrieval for in vitro fertilization.
