**BQ-V14** is a 14-day oral, vonoprazan-based bismuth quadruple regimen evaluated for first-line eradication of *Helicobacter pylori* infection. It consists of vonoprazan, bismuth tripotassium dicitrate, tetracycline, and metronidazole administered as separate active medicines. Vonoprazan provides potent potassium-competitive inhibition of gastric acid secretion, increasing intragastric pH and supporting antibiotic activity; tetracycline and metronidazole provide antibacterial activity, while bismuth has direct anti-*H. pylori* activity including urease inhibition. The regimen was studied by National Taiwan University Hospital in the randomized NCT04713670 trial.
