Overview

BR4044 is a preclinical-stage small molecule drug candidate developed by Boulder BioScience for the treatment of severe traumatic brain injury (TBI), moderate-to-severe TBI contusion, and acute ischemic stroke (AIS) undergoing thrombectomy. The active pharmaceutical ingredient is 3,3'-diindolylmethane (DIM), a naturally occurring bis-indole. BR4044 is formulated as an intravenous nanoscale emulsion/nano-suspension designed for emergency administration. It acts as a pleiotropic anti-inflammatory and neuroprotective agent through multiple mechanisms: it inhibits fibrin accumulation in thromboses, reduces extracellular vesicle (EV) release to protect the blood-brain barrier, inhibits neutrophil extracellular traps (NETs) and neutrophil activation, and modulates the aryl hydrocarbon receptor (AhR). The drug is intended to prevent microbleeding, reduce brain edema, and limit contusion formation following acute neurological injury.
