Overview

Brentuximab vedotin + doxorubicin + vinblastine + dacarbazine is a four-drug combination regimen used primarily as first-line therapy for classical Hodgkin lymphoma (cHL), especially in advanced stages. Brentuximab vedotin is an antibody-drug conjugate targeting CD30 on malignant cells, delivering the cytotoxic agent monomethyl auristatin E (MMAE) to induce apoptosis. Doxorubicin is an anthracycline antibiotic that intercalates DNA and inhibits topoisomerase II, leading to DNA damage and cell death. Vinblastine disrupts microtubule formation, inhibiting mitosis. Dacarbazine acts as an alkylating agent causing DNA cross-linking and apoptosis. This combination omits bleomycin (as in the traditional ABVD regimen) to reduce pulmonary toxicity while maintaining or improving efficacy[1][4][5][7].
