Overview

Brigimadlin + ezabenlimab is a combination therapy under clinical investigation for advanced solid tumors, notably including biliary tract cancer (BTC). Brigimadlin (BI 907828) is a highly potent, oral MDM2–p53 antagonist that inhibits the negative regulator interaction between TP53 (tumor suppressor protein) and MDM2, thereby stabilizing and activating p53, which induces cell-cycle arrest and apoptosis in tumor cells. Ezabenlimab (BI 754091) is an anti–programmed cell death protein 1 (PD-1) monoclonal antibody checkpoint inhibitor that blocks PD-1, promoting T-cell activation and anti-tumor immune response. The combination leverages brigimadlin’s direct tumor suppressive effects and ezabenlimab’s immunomodulatory checkpoint inhibition. The combination has shown encouraging preliminary efficacy with a manageable safety profile in early-phase studies, particularly in patients with *MDM2*-amplified, *TP53* wild-type tumors[1][2][3][5].
