Overview

BRIMOCHOL is a proprietary fixed-dose combination ophthalmic solution containing carbachol (a cholinergic agent) and brimonidine tartrate (an alpha-2 adrenergic agonist). It is developed as a pupil-modulating eye drop for the treatment of presbyopia. The mechanism of action involves inducing miosis (pupil constriction) through carbachol and reducing ocular redness via the vasoconstrictive properties of brimonidine. Together, these agents create a “pinhole effect,” which improves depth of focus and sharpens both near and distant vision without significant loss in distance visual acuity. Clinical trials have demonstrated that this combination provides greater efficacy and duration compared to monotherapy with either component alone, with effects lasting over eight hours. The formulation is preservative-free and intended for once-daily use[2][3][4][5][7].
