BRIMOCHOL F is an investigational topical ophthalmic solution developed by Visus Therapeutics for the treatment of presbyopia, the age-related loss of near vision. It is a fixed-dose combination of carbachol, a cholinergic miotic and muscarinic agonist, and brimonidine tartrate, an alpha-2 adrenergic agonist. The formulation works by inducing miosis (pupillary constriction) through carbachol's action on the iris sphincter muscle, creating a "pinhole effect" that increases the depth of focus. Brimonidine tartrate is included to enhance the miotic effect, prolong the duration of action, and mitigate potential side effects such as ciliary muscle contraction-induced brow ache or hyperemia. In the Phase 2 VIVID study (NCT04774237), BRIMOCHOL F met its primary endpoint for near visual acuity improvement. Following this study, the developer prioritized a preservative-free version, BRIMOCHOL PF, for Phase 3 development.
