Brivoligide (formerly known as AYX-1) is an investigational oligonucleotide drug developed for the reduction of acute post-surgical pain. It is a 23 base-pair double-stranded DNA transcription factor decoy oligonucleotide administered intrathecally at the time of surgery. Brivoligide acts by inhibiting early growth response protein 1 (EGR1), a transcription factor transiently upregulated in neurons critical to pain sensation following surgical trauma. By blocking EGR1 activity in the dorsal root ganglia and spinal cord during this period, brivoligide disrupts gene expression waves that lead to neuronal sensitization and prolonged postoperative pain. Clinical studies have shown that a single administration can accelerate recovery from pain and reduce opioid use after surgery in patients at high risk for increased or prolonged postoperative pain[2][5][6][7].
