Bromhexine is a small molecule mucolytic agent used to reduce the viscosity of mucus in the airways and enhance its clearance. It is primarily indicated for respiratory conditions associated with abnormal or excessive mucus secretion such as bronchitis, influenza, and productive cough. Introduced in 1963 and derived from the *Adhatoda vasica* plant, bromhexine acts by breaking down mucopolysaccharide fibers within mucus and stimulating secretions from both goblet cells and submucosal glands. This results in thinner mucus that can be more easily cleared by ciliary action[1][2][6][8]. Additionally, recent studies suggest that bromhexine inhibits transmembrane protease serine 2 (TMPRSS2), which may block viral entry into lung cells—a mechanism of interest for diseases like COVID-19[2]. Its main metabolite is ambroxol.
