Bromotheophylline is a xanthine derivative used primarily as a mild diuretic. It is the active component of pamabrom, which is commonly combined with acetaminophen for the relief of temporary water weight gain, bloating, swelling, and a full feeling associated with premenstrual and menstrual periods. Its mechanism of action involves increasing renal tubule permeability, enhancing glomerular filtration rate, and inhibiting sodium reabsorption in the proximal tubule. Bromotheophylline may also act as an adenosine receptor A1 antagonist and inhibit adenosine reuptake in the nephron, contributing to its diuretic effect. The drug’s therapeutic effects are mainly attributed to increased urinary excretion of sodium and chloride ions.
