Overview

BST-CarGel is a chitosan-based biotherapeutic scaffold designed for the repair of cartilage defects. Originally developed by the Canadian firm BioSyntech and later acquired by Piramal Healthcare (now Piramal Pharma), the product consists of a liquid chitosan-glycerol phosphate solution that is mixed with a patient's autologous blood at the point of care. This mixture is delivered into a cartilage lesion following a microfracture procedure, where it undergoes rapid gelation to stabilize the blood clot. By providing a structural scaffold, BST-CarGel facilitates the recruitment and retention of bone marrow-derived mesenchymal stem cells, promoting the regeneration of hyaline-like cartilage rather than the inferior fibrocartilage typically formed after standard microfracture surgery. The product received CE Mark approval in Europe in 2012 for the treatment of cartilage lesions in the knee.
