BTCA is a combination chemotherapy regimen primarily investigated for the treatment of recurrent glioblastoma multiforme (GBM). The acronym stands for its four constituent agents: **B**ortezomib (a proteasome inhibitor), **T**emozolomide (an alkylating agent), **C**CNU (lomustine, an alkylating agent), and **A**driamycin (doxorubicin, an anthracycline). This regimen is designed to overcome therapeutic resistance in high-grade gliomas by combining agents with distinct mechanisms of action. Bortezomib, in particular, is thought to sensitize tumor cells to the cytotoxic effects of DNA-damaging agents and may also modulate the immune microenvironment, including enhancing the activity of natural killer (NK) cells. Recent research suggests that NK cell-derived granzyme B (GZMB) can further sensitize glioblastoma cells to radiotherapy by blocking SDC1-mediated autophagosome maturation, a pathway that components of the BTCA regimen may interact with to improve clinical outcomes.
