BUCY2 is a multi-agent conditioning regimen used prior to hematopoietic stem cell transplantation, particularly in the context of haploidentical transplants for hematologic malignancies such as high-risk acute myeloid leukemia. In the protocol utilized by the First Affiliated Hospital of Xi'an Jiaotong University, the regimen consists of a combination of the DNA alkylating agents busulfan (BU), cyclophosphamide (CTX), and semustine (me-CCNU), the antimetabolite cytarabine (Ara-C), and the immunosuppressive anti-thymocyte globulin (ATG). This combination is designed to achieve profound myeloablation and immunosuppression to eradicate residual disease and prevent graft rejection or graft-versus-host disease. Clinical studies at the institution have evaluated BUCY2 as a standard-of-care control arm against modified regimens like MA-BUCY2, which incorporates mitoxantrone liposomes.
