This investigational combination regimen consists of the probiotic strain *Lactobacillus casei* Zhang and enteric-coated budesonide capsules. It is currently being evaluated in a Phase 4 clinical trial (NCT06528015) for the treatment of primary IgA nephropathy (IgAN). Budesonide is a locally acting corticosteroid that targets gut-associated lymphoid tissue (GALT) to reduce the production of galactose-deficient IgA1 (Gd-IgA1), a key driver of IgAN pathogenesis. The addition of *Lactobacillus casei* Zhang, a live biotherapeutic agent, is intended to modulate the gut microbiome and restore intestinal homeostasis, potentially enhancing the therapeutic effect of budesonide on mucosal immunity. The trial is led by Professor Gang Xu of Tongji Hospital in collaboration with the China Primary Health Care Foundation.
