Bufei Huoxue (BFHX) is a traditional Chinese medicine herbal formulation available as an oral capsule. It is composed of three main herbal ingredients: Astragalus mongholicus (Milkvetch Root), Paeonia anomala subsp. veitchii (Red Peony Root), and Cullen corylifolium (Malaytea Scurfpea Fruit/psoralen)[2][3][4]. The formulation is used primarily in China for the treatment of chronic obstructive pulmonary disease (COPD), convalescent COVID-19 infection ("long COVID"), pulmonary heart disease with Qi deficiency and blood stasis syndrome, and asthma[2][3][4][5]. Mechanistically, BFHX exhibits anti-inflammatory effects by inhibiting activation of the NLRP3 inflammasome and macrophages in lung tissue[1][2], reducing cytokine levels and inflammatory cell infiltration. Clinical studies suggest it improves respiratory symptoms, exercise tolerance, quality of life scores such as St. George’s Respiratory Questionnaire and Fatigue Assessment Inventory, 6-minute walk distance performance, lung function parameters on CT imaging[6], reduces blood viscosity[8], and may regulate gut-lung axis inflammation via modulation of the microbiome[2]. It has demonstrated efficacy in both acute exacerbations and recovery phases of pulmonary diseases including post-acute COVID-19 syndrome ("long COVID")[5][6]. Adverse events are rare but may include dry mouth or mild gastrointestinal symptoms; no significant safety concerns have been reported in clinical trials[9].
