Bufei Yishen Granules (补肺益肾颗粒) is a traditional Chinese medicine (TCM) herbal formulation designed to "tonify the lungs and benefit the kidneys." It is composed of multiple herbs including Astragalus membranaceus (Huang Qi), Cornus officinalis (Shan Zhu Yu), Lycium barbarum (Gou Qi Zi), Schisandra chinensis (Wu Wei Zi), Fritillaria thunbergii (Zhe Bei Mu), Perilla frutescens seed (Zi Su Zi), Citrus reticulata peel (Chen Pi), Paeonia lactiflora root (Bai Shao), Epimedium brevicornum Maxim. (Yin Yang Huo), Stemona japonica or Cynanchum stauntonii root/tuber ("Zi Jin Niu" or Bai Bu), Lumbricus ("Di Long"), and Panax ginseng root ("Ren Shen")[1]. The granule form is produced via water extraction from these herbs. The primary clinical indication for Bufei Yishen Granules is chronic obstructive pulmonary disease (COPD). Clinical studies have shown that it can improve quality of life and reduce inflammation in patients with stable COPD[2][4]. Mechanistic research suggests that its effects are mediated by modulation of autophagy pathways—specifically upregulation of Sirt1/AMPK/FOXO3 signaling, increased expression of autophagy-related proteins such as LC3-B and Beclin-1, reduction in inflammatory cytokines like TNF-α, IL-8, IL-1β, and inhibition of apoptosis markers such as Bax and Caspase-3[3]. These actions contribute to improved airway epithelial barrier function and reduced lung inflammation.
