Bupi Yishen (BYF) is a patented traditional Chinese herbal compound used clinically to treat chronic kidney disease (CKD) and delay its progression. It has demonstrated renoprotective effects by alleviating renal impairment and fibrosis, improving renal function, and reducing systemic inflammation. Its mechanisms involve modulating the gut microbiota, particularly enriching *Akkermansia muciniphila*, activating group 3 innate lymphoid cells (ILC3s), upregulating interleukin-22 (IL-22) expression, and elevating retinoic acid levels to enhance intestinal barrier function. BYF also restores renal energy metabolism, including fatty acid oxidation, glucose metabolism, and mitochondrial oxidative phosphorylation. Furthermore, it exerts anti-fibrotic effects by inhibiting the PI3K/AKT signaling pathway, downregulating Toll-like receptor 4 (TLR4) and NF-κB signaling, and inhibiting the tryptophan-kynurenic acid-aryl hydrocarbon receptor (AhR) pathway. Clinical trials have shown BYF to be effective in slowing renal function decline in non-diabetic stage 4 CKD patients, with comparable safety to losartan.
