Bupivacaine + adrenaline is a combination drug product consisting of the long-acting amide-type local anesthetic bupivacaine and the sympathomimetic vasoconstrictor adrenaline (epinephrine). Bupivacaine functions by binding to the intracellular portion of voltage-gated sodium channels, effectively blocking sodium influx into nerve cells and preventing the initiation and conduction of nerve impulses. The addition of adrenaline induces local vasoconstriction, which serves to decrease the rate of systemic absorption of bupivacaine, thereby prolonging the duration of the analgesic effect and reducing the risk of systemic toxicity. In clinical research conducted by the Medical University of Warsaw, this combination is utilized in ultrasound-guided regional anesthesia techniques, specifically Transversus Abdominalis Plane Block (TAPB) and Quadratus Lumborum Block (QLB), to manage perioperative and postoperative pain in patients undergoing kidney transplantation.
