In this protocol, patients receive a pericapsular nerve group (PENG) block using the local anesthetic bupivacaine, supplemented by a systemic (intravenous) dose of dexamethasone. Bupivacaine is a long-acting local anesthetic that functions by binding to and blocking voltage-gated sodium channels, thereby inhibiting nerve impulse generation and conduction. Dexamethasone, a potent synthetic glucocorticoid, is used as an adjuvant to prolong the duration of the sensory block and provide systemic anti-inflammatory effects, which helps in reducing postoperative pain scores and opioid consumption in patients undergoing femoral neck fracture surgery.
