This combination therapy consists of the long-acting amide local anesthetic bupivacaine and the synthetic corticosteroid dexamethasone. It is being investigated by Tanta University for use in ultrasound-guided regional anesthesia, specifically the erector spinae plane block (ESPB) and thoracic paravertebral block (TPVB), to manage acute pain in patients with thoracic herpes zoster. Bupivacaine provides analgesia by inhibiting voltage-gated sodium channels, thereby blocking nerve conduction. Dexamethasone is added as an adjuvant to enhance and prolong the analgesic effect of the local anesthetic and to reduce the inflammatory response associated with viral nerve damage, with the ultimate goal of preventing the development of post-herpetic neuralgia.
