This is a combination of three drugs—bupivacaine, dexamethasone, and dexmedetomidine—used primarily as an adjuvant mixture for regional anesthesia, especially in spinal or peripheral nerve blocks. - **Bupivacaine** is a long-acting local anesthetic that works by blocking voltage-gated sodium channels on neuronal membranes, thereby inhibiting nerve impulse transmission and providing sensory and motor block. - **Dexamethasone** is a synthetic glucocorticoid corticosteroid with potent anti-inflammatory properties; when used as an adjuvant in regional anesthesia, it prolongs analgesia likely through vasoconstriction (reducing local anesthetic absorption), inhibition of potassium channels on nociceptive C-fibers, and suppression of inflammatory responses. - **Dexmedetomidine** is a highly selective alpha-2 adrenergic receptor agonist that provides sedative and analgesic effects by inhibiting the release of nociceptive neurotransmitters in the central nervous system. The combination has been shown to prolong the duration of sensory block and postoperative analgesia compared to bupivacaine alone or with single adjuvants[1][7][10].
