Bupivacaine + neostigmine is a combination of two pharmaceutical agents used primarily as an adjunct for regional anesthesia and postoperative analgesia. Bupivacaine is an amide-type local anesthetic that works by blocking voltage-gated sodium channels in neuronal membranes, thereby inhibiting nerve impulse conduction and producing local or regional anesthesia[6]. Neostigmine is a reversible cholinesterase inhibitor that increases the concentration of acetylcholine at synaptic junctions by inhibiting acetylcholinesterase, leading to enhanced stimulation of muscarinic and nicotinic receptors[8]. When administered together via caudal or intrathecal routes, this combination has been shown to provide superior and prolonged postoperative analgesia compared to either agent alone, with reduced need for supplementary analgesics. The mechanism involves both direct nerve blockade (bupivacaine) and potentiation of spinal cholinergic pathways (neostigmine), resulting in synergistic antinociceptive effects[1][3][5].
