This intervention is a therapeutic pudendal nerve block developed and studied by the University of California, Irvine, specifically for the treatment of pelvic floor tension myalgia (PFTM), also known as myofascial pelvic pain syndrome. The block consists of a combination of bupivacaine, a long-acting amide local anesthetic, and triamcinolone, a synthetic corticosteroid, buffered with sodium bicarbonate to reduce injection pain and potentially accelerate the onset of action. Bupivacaine provides regional anesthesia by inhibiting voltage-gated sodium channels, thereby blocking nerve conduction, while triamcinolone provides anti-inflammatory effects to reduce nerve irritation and associated muscle tension. In the clinical trial NCT00928564, this combination was administered weekly in conjunction with pelvic floor physical therapy to evaluate its efficacy in improving pain scores and pelvic floor muscle strength in women with chronic pelvic pain.
