bupivacaine + triamcinolone acetonide

bupivacaine + triamcinolone acetonide is an investigational combination therapy being evaluated by the Foundation for Orthopaedic Research and Education (FORE) for the management of knee osteoarthritis. The combination consists of the amide-type local anesthetic bupivacaine and the synthetic corticosteroid triamcinolone acetonide (Kenalog). Administered via intra-articular injection, bupivacaine provides immediate, short-term analgesia by blocking voltage-gated sodium channels and inhibiting nerve conduction. Triamcinolone acetonide acts as a glucocorticoid receptor agonist, offering sustained anti-inflammatory effects to alleviate joint pain and swelling. The Phase 1 clinical trial aims to optimize the volume and concentration of bupivacaine mixed with the corticosteroid to maximize immediate pain relief during the injection process while maintaining the long-term therapeutic efficacy of the steroid treatment in patients with Kellgren-Lawrence Stage II or III knee osteoarthritis.
