This combination therapy consists of bupivacaine hydrochloride, a long-acting amide-type local anesthetic, and methylprednisolone acetate (marketed as Depo-Medrol), a synthetic glucocorticoid. Bupivacaine works by binding to the intracellular portion of voltage-gated sodium channels, which inhibits sodium influx and prevents the generation and conduction of nerve impulses, providing regional anesthesia. Methylprednisolone acetate is added to the local anesthetic to prolong the duration of the nerve block and provide localized anti-inflammatory effects, which can reduce postoperative pain and the subsequent need for systemic opioids. This specific combination is being evaluated by researchers at Menoufia University for use in ultrasound-guided genicular nerve blocks, often in conjunction with adductor canal blocks, to manage pain and facilitate rehabilitation in patients undergoing reconstructive knee surgeries such as total knee arthroplasty (TKA).
