Drug intelligence / Profile preview
This is a combination of three pharmaceutical agents—buprenorphine, clonidine, and dexamethasone—used as adjuvants to local anesthetics in regional anesthesia, particularly for peripheral nerve blocks. - **Buprenorphine** is a partial agonist at the mu-opioid receptor and antagonist at the kappa-opioid receptor, providing prolonged analgesia with reduced risk of respiratory depression compared to full opioid agonists. - **Clonidine** is an alpha-2 adrenergic receptor agonist that enhances analgesia by inhibiting nociceptive transmission in the dorsal horn of the spinal cord. - **Dexamethasone** is a synthetic glucocorticoid that exerts anti-inflammatory effects via activation of glucocorticoid receptors and modulation of gene expression, which can prolong block duration and reduce postoperative pain and inflammation. When combined as perineural additives (injected around nerves), these agents are used to prolong the duration of analgesia from local anesthetic nerve blocks while minimizing motor blockade. This multimodal approach has been studied primarily in orthopedic surgeries such as shoulder arthroplasty or upper extremity procedures[1][2][4]. The combination does not have a unique brand name or fixed-dose commercial product; it is prepared by clinicians for off-label use based on clinical protocols.
