Buprenorphine + fentanyl is a combination of two synthetic opioids: buprenorphine, a partial agonist at the mu-opioid receptor used in pain management and opioid use disorder, and fentanyl, a highly potent full mu-opioid receptor agonist primarily used for severe pain and anesthesia. This combination is not an established, FDA-approved pharmaceutical product but may represent the concomitant or sequential use of these drugs in clinical or illicit contexts. Buprenorphine functions as a partial agonist with a ceiling effect for respiratory depression, making it safer regarding overdose risk, and also antagonizes kappa-opioid receptors. Fentanyl is a full agonist at the mu-opioid receptor, providing high-potency analgesia but with a significant risk of respiratory depression and overdose. The co-use of these agents is clinically notable: higher doses of buprenorphine may be needed for opioid use disorder patients who are exposed to fentanyl due to increased tolerance and downregulation of opioid receptor expression. Combined use of both drugs increases the risk of additive central nervous system (CNS) and respiratory depression, and their combination is associated with significant risk in non-medical settings.
