Overview

A combination therapy consisting of **burixafor** (GPC-100), **propranolol**, and **granulocyte colony-stimulating factor (G-CSF)** is under clinical investigation as a novel regimen for **mobilizing hematopoietic stem and progenitor cells (HSPCs)** in patients at risk of poor stem cell mobilization, such as those with lymphoma or multiple myeloma. - **Burixafor** is a small molecule antagonist of CXCR4 that disrupts the CXCR4-CXCL12 axis, facilitating the release of HSPCs from the bone marrow into peripheral blood. - **Propranolol** is a non-selective beta-adrenergic receptor antagonist (beta blocker), co-targeting the β2 adrenergic receptor pathway, shown to synergistically enhance stem cell mobilization when combined with burixafor, as both CXCR4 and β2 adrenergic receptors are co-expressed and interact to regulate immune cell trafficking. - **G-CSF** (recombinant granulocyte colony-stimulating factor, including filgrastim) stimulates proliferation of neutrophils and further augments stem cell release. Preclinical and early clinical studies show the triple combination provides superior mobilization of primitive, functional HSPCs compared to standard regimens, and may benefit populations intolerant of G-CSF or with mobilization failure. The regimen is being evaluated in a Phase II trial for autologous stem cell transplantation in multiple myeloma patients[1][2][4][5][7].
