MBU + CY (modified Busulfan/Cyclophosphamide) is an intensified myeloablative conditioning regimen used prior to allogeneic hematopoietic stem cell transplantation (allo-HSCT). Developed and primarily utilized by researchers at Peking University People's Hospital, this regimen is a modification of the standard BuCy protocol, designed to optimize the balance between anti-leukemic activity and transplant-related mortality. It consists of busulfan, a bifunctional alkylating agent that causes DNA cross-linking, and cyclophosphamide, a nitrogen mustard that interferes with DNA replication and transcription. In clinical practice, particularly for high-risk or relapsed/refractory acute leukemia, this regimen is frequently combined with epigenetic modulators like decitabine to enhance the graft-versus-leukemia effect and reduce the risk of relapse by reprogramming leukemic cells and modulating T-cell responses.
