This therapeutic regimen is a specific myeloablative conditioning and graft-versus-host disease (GVHD) prophylaxis protocol developed and evaluated by the Masonic Cancer Center at the University of Minnesota. It is designed for patients undergoing allogeneic hematopoietic stem cell transplantation (HCT) for various hematological malignancies who cannot receive total body irradiation (TBI). The conditioning phase utilizes a combination of busulfan and fludarabine to achieve myeloablation and eradicate malignant cells. Following the transplant, a triple-drug prophylaxis regimen is employed to prevent GVHD, consisting of high-dose post-transplant cyclophosphamide (PTCy), the calcineurin inhibitor tacrolimus, and the antimetabolite mycophenolate mofetil (MMF). This approach aims to provide effective disease control and stable engraftment while minimizing the toxicities associated with radiation-based conditioning.
