Butenyl aminolevulinic acid (Butenyl ALA) is a lipophilic ester derivative of 5-aminolevulinic acid (ALA) developed by Seoul National University Hospital for the treatment of acne vulgaris. As a prodrug, it is converted intracellularly into protoporphyrin IX (PpIX), a potent photosensitizer. While ALA and its derivatives are typically used in photodynamic therapy (PDT) requiring a specific light source to induce reactive oxygen species and target sebaceous glands, this 1.5% cream formulation is specifically being evaluated for its efficacy and safety without exogenous light irradiation. The butenyl ester modification is designed to enhance skin penetration and lipophilicity compared to standard ALA, potentially allowing for therapeutic effects through ambient light activation or direct biochemical modulation of the pilosebaceous unit.
