Butoconazole is an imidazole antifungal agent primarily used as a vaginal cream for the local treatment of vulvovaginal candidiasis (yeast infections) caused by *Candida albicans*. It works by inhibiting the synthesis of ergosterol, a key component of fungal cell membranes. This is achieved through inhibition of the enzyme lanosterol 14-alpha demethylase (a cytochrome P450 enzyme), leading to altered membrane permeability and ultimately growth inhibition or death of the fungus[4][6][8]. Butoconazole may also exhibit some antibacterial activity against gram-positive organisms. The drug is approved for use in adults and has demonstrated clinical efficacy in treating vaginal yeast infections[1][4][5].
