Buyang Huanwu decoction (BYHW) is a classic Traditional Chinese Medicine (TCM) herbal formula traditionally used to treat cardiovascular and cerebrovascular diseases, particularly ischemic heart failure (IHF) and stroke. The specific formulation evaluated by Zhu Mingjun at the First Affiliated Hospital of Henan University of Chinese Medicine consists of eight key ingredients: *Radix Astragali* (Huangqi), *Radix Angelicae Sinensis* (Danggui), *Radix Paeoniae Rubra* (Chishao), *Rhizoma Ligustici Chuanxiong* (Chuanxiong), *Salvia miltiorrhiza* (Danshen), *Semen Persicae* (Taoren), *Flos Carthami* (Honghua), and *Lumbricus* (Dilong). BYHW is indicated for patients presenting with 'Qi deficiency and blood stasis' syndrome according to TCM theory. Clinical research has demonstrated its potential to improve New York Heart Association (NYHA) classification, reduce NT-proBNP levels, and modulate energy metabolism and coagulation function in patients with IHF. Its mechanism of action involves multi-target modulation of angiogenesis, anti-inflammatory pathways, and myocardial energy metabolism.
