Buyang Huanwu granules (BYHW) is a traditional Chinese medicine (TCM) formulation derived from the classic Buyang Huanwu decoction (BHD), which has been used for centuries to treat cardiovascular and cerebrovascular diseases. The formulation consists of seven key herbal ingredients: *Astragalus mongholicus* (Huangqi), *Angelica sinensis* (Danggui), *Paeonia lactiflora* (Chishao), *Lumbricus rubellus* (Dilong), *Ligusticum chuanxiong* (Chuanxiong), *Carthamus tinctorius* (Honghua), and *Prunus persica* (Taoren). It is primarily used to treat stable angina pectoris (SAP) in coronary heart disease and ischemic stroke, particularly in patients presenting with the TCM syndrome pattern of qi deficiency and blood stasis (QDBS). BYHW works by protecting vascular endothelial function, delaying atherosclerosis, improving hemorheology, reducing blood lipids, and exerting anti-inflammatory and antithrombotic effects.
