Overview

BX2 is a **trial-arm designation for a reduced-intensity allogeneic hematopoietic stem-cell-transplant conditioning regimen**, not a distinct single molecular entity. In the Institut Paoli-Calmettes phase II AAA study, BX2 denotes the two-day intravenous busulfan arm: busulfan 3.2 mg/kg/day for 2 days, combined with fludarabine and rabbit anti-thymocyte globulin before allogeneic transplantation in adults with poor-risk acute myeloid leukemia or myelodysplastic syndrome. Busulfan produces DNA cross-linking and cytotoxic myeloablation; fludarabine is a purine-nucleoside analog that suppresses DNA synthesis and provides immunosuppression; rabbit anti-thymocyte globulin depletes T cells to support engraftment and reduce graft-versus-host disease risk.
