**BX3** is an experimental allogeneic hematopoietic stem-cell-transplant conditioning regimen, not a distinct single molecular drug. In the Institut Paoli-Calmettes phase II study NCT01985061, BX3 denotes intravenous busulfan administered at 3.2 mg/kg/day for 3 days, combined with fludarabine and thymoglobuline before matched related- or unrelated-donor allogeneic transplantation in patients with poor-prognosis myeloid malignancies, including acute myeloid leukemia and myelodysplastic syndrome. Busulfan provides myeloablative DNA alkylation, fludarabine adds lymphodepleting purine-antimetabolite activity, and thymoglobuline provides polyclonal T-cell immunosuppression to facilitate engraftment and reduce graft rejection.
