BXOS110 is a recombinant human prourokinase (rhpro-UK) developed by Biocells Therapeutics for the treatment of acute ischaemic stroke (AIS). As a second-generation thrombolytic agent, prourokinase is a single-chain urokinase-type plasminogen activator (scu-PA) that exhibits high fibrin selectivity. It remains relatively inactive in the systemic circulation, which helps minimize the risk of systemic fibrinogen depletion and bleeding complications. Upon binding to a fibrin clot, BXOS110 is converted by plasmin into active two-chain urokinase (tcu-PA), which then localizedly activates plasminogen to plasmin, leading to the dissolution of the thrombus. The drug is currently being evaluated in Phase II clinical trials for administration within a narrow therapeutic window (typically within 3 hours of stroke onset) to restore blood flow and reduce overall disability in patients.
