Overview

BYON5667 + SYD985 is a combination therapeutic regimen developed by Byondis B.V. to mitigate the ocular toxicities associated with the antibody-drug conjugate (ADC) SYD985 (trastuzumab duocarmazine). SYD985 is a HER2-targeting ADC that utilizes a duocarmycin-based DNA-alkylating payload (seco-DUBA) linked via a cleavable valine-citrulline linker. While effective in treating HER2-expressing cancers such as breast and endometrial cancer, SYD985 frequently causes ocular adverse events, including keratitis and conjunctivitis, likely due to off-target uptake of the payload in the eye. BYON5667 is a prophylactic ophthalmic formulation of sodium thiosulfate designed to neutralize the cytotoxic payload within the ocular environment, thereby reducing the severity and incidence of these side effects. This combination is currently being evaluated in Phase 1/2 clinical trials.
