Overview

C-11 PiB (Carbon-11 labeled Pittsburgh Compound B) is a small molecule radiopharmaceutical used as a positron emission tomography (PET) imaging tracer. It is a derivative of thioflavin T that specifically binds with high affinity to fibrillar beta-amyloid plaques in the brain, which are a primary pathological hallmark of Alzheimer's disease. Due to the short 20-minute half-life of the Carbon-11 isotope, the tracer must be produced on-site using a cyclotron. It is widely considered the gold-standard research tracer for quantifying amyloid burden in vivo. Mayo Clinic utilizes C-11 PiB in extensive longitudinal neuroimaging studies, such as the Mayo Clinic Study of Aging, to track disease progression, differentiate Alzheimer's from other dementias, and evaluate the efficacy of amyloid-targeting therapeutics.
