C-13 labeled oxalate is a diagnostic research tool consisting of oxalic acid labeled with the stable isotope carbon-13. It is primarily utilized by the Mayo Clinic and other research institutions to evaluate enteric (intestinal) oxalate absorption in patients with hyperoxaluria and kidney stone disease. The compound is typically administered orally, often as the disodium salt of [13C2]oxalic acid in capsule form or mixed into food. By measuring the amount of labeled oxalate excreted in the urine over a 24-hour period using gas chromatography-mass spectrometry (GC-MS), clinicians can calculate intestinal absorption rates. This diagnostic test is essential for differentiating between metabolic hyperoxaluria, which involves increased endogenous production, and hyperabsorptive hyperoxaluria, which results from excessive intestinal absorption. As a stable isotope tracer, it is a non-radioactive diagnostic agent rather than a therapeutic intervention.
