C-TIL (also known as SMART-TIL) is an autologous cell therapy developed by MedGene Therapeutics that utilizes neoantigen-specific CD8+ T cells isolated from a patient's peripheral blood. Unlike traditional tumor-infiltrating lymphocyte (TIL) therapies that require surgical resection of a tumor mass to harvest immune cells, C-TIL identifies and selects mutation-specific T cells circulating in the blood. These cells are then expanded ex vivo and re-infused into the patient. This approach aims to provide the benefits of TIL therapy—specifically the ability to target personalized tumor neoantigens—while avoiding the risks and limitations associated with invasive surgery and the requirement for accessible tumor tissue.
