Overview

CAB-HER2 is a conditionally active bispecific antibody (CAB-bispecific) developed by Himalaya Therapeutics, utilizing technology licensed from BioAtla. It is designed as a T-cell engager that simultaneously targets Human Epidermal Growth Factor Receptor 2 (HER2) on tumor cells and the CD3 subunit of the T-cell receptor complex on T lymphocytes. The "Conditionally Active Biologic" (CAB) nature of the molecule allows it to bind its targets preferentially in the acidic microenvironment of the tumor (low pH) while remaining relatively inactive at the physiological pH of healthy tissues. This mechanism is intended to expand the therapeutic window by reducing systemic toxicities, such as "on-target, off-tumor" effects, which are common challenges for HER2-targeted therapies and potent T-cell engagers. By bridging T cells to HER2-positive cancer cells, CAB-HER2 facilitates the formation of an immunological synapse, leading to T-cell activation and the directed lysis of the malignant cells.
